Mary Gosciminski, C-TAGME – GME Facility Director

Mary has 13 years of experience in the GME world. She enjoys working with residents and seeing them grow into successful practicing physicians. She is happy to start her next chapter here at Northwest Medical Center working with the Medicine Coordinators.

Anastasia Sokhan, M.D. – Associate Program Director (Family Medicine)

Dr. Sokhan completed medical school at Moscow State University Faculty of Medicine, in Moscow, Russia. She completed her training with the Family Medicine residency at Baptist Health – University of Arkansas for Medical Sciences in North Little Rock, AR. She is board certified by the American Board of Family Medicine.

Anthony DeMarco, M.D. – Core Faculty (Family Medicine)

Dr. DeMarco completed medical school at the University of Cincinnati College of Medicine in Ohio and continued on with his residency at the Ohio State University. He is board certified by the American Board of Family Medicine.

Jonny Neilson, M.D. – Core Faculty (Family Medicine, Pediatrics)

Dr. Neilson completed medical school at the University of Alabama School of Medicine in Birmingham and his pediatrics residency at the University of Arizona – Tucson. He is board certified by the American Board of Pediatrics. He fell in love with the desert beauty of Tucson.

Jason Patel, M.D. – Core Faculty (Family Medicine)

Dr. Patel completed medical school at the University of Arizona College of Medicine -Tucson, and continued on with his Family Medicine residency at the University of Arizona College of Medicine at South Campus in Tucson. He is board certified by the American Board of Family Medicine.

Prido Jose Polanco, Jr., M.D. – Core Faculty (Family Medicine)

After completing my medical training in Houston Texas, where I gained experience caring for patients from diverse backgrounds, I returned to Arizona to focus on community-based family medicine and teaching. That early inspiration continues to guide me today as I mentor family medicine and internal medicine resident physicians in Northwest’s graduate medical education program. My training and enthusiasm for education means that I combine compassionate patient care with the development of strong future clinicians.

Melissa Horne, MHA – Residency Program Coordinator (Family Medicine)

I’m a first-generation Tucson native. I first discovered the GME world in 2018 working at the Veterans Administration Hospital here in Tucson, Arizona where I fell in love! I had the pleasure of working with medical students, residents and fellows of all specialties. I have been with Northwest Family Medicine residency since the summer of 2025 and wouldn’t have it any other way!
